Title: Anyone Have A Oop Sheet They Can Scan For Me?

im just looking to see what they check. ive been told more than once but never seen the actual sheet and being the careful person i am thats just not good enough

if someone could help me out it would be much appreciated

here is a pic of mine for my 64.

printed above that is a

-check for any physical damage to windshield or prior body damage

-reason for inspection (first time registered in bc, notice & order, motorcycle, rebuilt from salvage, other...)

-aswell as personal info about owner , vehicle & shop doing the inspection.
